If you haven't completed a degree yet or you are pursuing your first degree then provide where you are enrolled and what your current major is.

Whether you are a student trying to enter into a new profession or an already established professional trying to transition into a different field your past employment can be helpful to your mentor. Please provide past employment and types of positions held.

Provide highlights on special or notable projects you have been a part of or led.

Provide any professional and academic certifications that you have obtained.

Provide a list of specific skills and areas of expertise that you desire to learn or improve. This will aid in matching you with a mentor that can provide direction in accomplishing your goals.
